WHAT TO BRING
All athletes trying out need to bring proof of a Badger Region tryout membership, the USAV medical release form, the player information form, the concussion form (p.5-6), and $30 cash or check. Forms are linked below:

All families are asked to fill out the tournament conflict form and turn it in with your tryout forms. If chosen for a team, this form will be used to select the best tournaments for all players.
